Online ordering for the St. Demetrius (Ukrainian Catholic) Development Corporation 2012 Living Well Calendar Campaign is not permitted by the Alcohol and Gaming Commission of Ontario.

For each calendar purchased, you will receive one wall calendar and an official remittance slip. It is the responsibility of the purchaser (or giftee) to return the remittance slip to St. Demetrius (Ukrainian Catholic) Development Corporation, 60 Richview Road, Toronto, ON M9A 5E4 to be eligible for the prize draws. Draws will occur from January 6, 2012 to December 28, 2012 as outlined below.

Each official remittance slip calendar number will have a corresponding token number in the draws. After each winning token is drawn and recorded, it is returned to the drum and is eligible for all subsequent draws.

Only 2,450 calendars are printed.

Purchasers must be at least 18 years of age. Calendars cannot be purchased in the name of a minor.

The St. Demetrius (Ukrainian Catholic) Development Corporation 2012 Living Well Calendar Campaign is intended for Ontario residents only. Calendar buyers must be in Ontario at the time of purchase and calendars will only be mailed within Ontario.

Calendars make a perfect gift for the holidays! Orders received by December 1, 2011 will be delivered to the purchaser by December 24th. Should a purchaser wish to give a calendar to another person who is older than 18 years old (the giftee), it is the responsibility of the giftee to submit the remittance slip to the St. Demetrius (Ukrainian Catholic) Development Corporation at 60 Richview Road, Toronto, ON M9A 5E4 to be placed in the Living Well Calendar Campaign to be eligible for the prize draws. The name on the remittance stub must not be a minor under the age of 18. Prize cheques will be made to the name and contact information on the remittance slip.

Calendar purchases requiring delivery prior to December 24, 2011 must be ordered by December 1, 2011.

Calendars purchasers must be at least 18 years of age. Calendars must be purchased and delivered within Ontario. Alcohol and Gaming Commission regulations prohibit the following individuals and all of their immediate family members residing in the same household from purchasing a calendar: St. Demetrius (Ukrainian Catholic) Development Corporation Board of Directors and employees, and members of licensed gaming suppliers providing services to the lottery program.

Lottery Calendars are NON -TAX DEDUCTIBLE in accordance with Canada Customs and Revenue Agency policies; charitable lotteries are prohibited from issuing tax receipts for lottery calendars. You CAN make a donation in addition to your calendar order and receive a tax receipt for the donation portion of your order. Complete the remittance slip located on the bottom of the rules and regulations page and mail the remittance slip back to St. Demetrius (Ukrainian Catholic) Development Corporation at 60 Richview Road, Toronto, Ontario M9A 5E4 before the January 6, 2012 draw date. If you have not received a calendar within 3 weeks of ordering, please contact the Development Office at 416-243-7653 ext 313 or by email fundraising@stdemetrius.ca to ensure that your order has been processed. Please provide your first and last name, your current address and your telephone number. Calendars cannot be refunded once the draws commence.

*Weekly, Monthly and Seasonal draws falling on weekend days or holidays will be drawn on the next business day.

Winners will be posted every week* on the St. Demetrius (Ukrainian Catholic) Development Corporation website at www.stdemetrius.ca/calendar or you can inquire by calling 416-243-7653 ext 313. All winners will be notified in writing and would be asked to complete a wavier form prior to the release of the cheque.

Please allow a minimum of 4 weeks for delivery of cheques. All prizes will be awarded to the name on the remittance slip. Winners agree to the use of their name and photo for publicity purposes by the St. Demetrius (Ukrainian Catholic) Development Corporation.

You will receive a letter and your cheque from the St. Demetrius (Ukrainian Catholic) Development Corporation. If you won and have not received the letter and cheque within 4 weeks of the day St. Demetrius (Ukrainian Catholic) Development Corporation receives the signed waiver, please call the Development Office at 416-243-7653 ext 313 or email: fundraising@stdemetrius.ca. Jackpot winner will require 2 pieces of ID.

All prizes will be delivered within 4 weeks from the day St. Demetrius (Ukrainian Catholic) Development Corporation receives the signed waiver. If you have not received your prize 4 weeks from the time the waiver has been submitted to the St. Demetrius (Ukrainian Catholic) Development Corporation, please call the Development Office 416-243-7653 ext 224, or email fundraising@stdemetrius.ca.

For those calendar lottery winners with delivery addresses outside of the province of Ontario that have won, the St. Demetrius (Ukrainian Catholic) Development Corporation will ship the prize to the winner name on the remittance slip as long as they are within Canada. Calendar Lottery winners with addresses outside of Canada that have won a prize must provide an alternate Canadian shipping address.

Unfortunately, the St. Demetrius (Ukrainian Catholic) Development Corporation 2012 Living Well Calendar Campaign cannot ship outside of Canada. Please allow delivery of 4 weeks for all prizes from the time the waiver has been submitted to the St. Demetrius (Ukrainian Catholic) Development Corporation.

The proceeds of the St. Demetrius (Ukrainian Catholic) Development Corporation 2012 Living Well Calendar Campaign will support the quality of life and exceptional services provided at the Ukrainian Canadian Care Centre (Care Centre), an ethno specific, 152-bed, long-term care home located in Toronto.

We are committed to achieving excellence in service delivery in a setting that is sensitive and respectful to the cultural and spiritual needs of everyone we serve.

When you come through the doors, you see how welcoming our environment is for residents and their families. You see how our multi disciplinary team of doctors, nurses and healthcare professionals care. Our team diagnoses and treats our residents with some of the most complex conditions facing seniors, and work together to meet the needs of each resident, who today have an average age of 86. Our staff is inspiring and their compassion sets the Care Centre apart.

We provide a special environment for individuals with difficult health care issues such as Alzheimer's and other cognitive disorders.

Since its opening in 1996, the Care Centre has provided quality care to more than 550 Ukrainian Canadian seniors. For Ukrainian Canadian seniors, the Care Centre is the best place to be. The proceeds of this calendar campaign will support our commitment to quality of life for our seniors.

St. Demetrius (Ukrainian Catholic) Development Corporation Operates the Ukrainian Canadian Care Centre and St. Demetrius Residence; a 15-story, 259-unit, independent seniors apartment for those 65 years and over.

The odds of winning are dependent on the number of Calendars that are sold. There are 2,450 calendars available to be sold. There are 69 cash prizes for a total prize pool value of $9,800.
