What is the Ukrainian Canadian Care Centre?
The Ukrainian Canadian Care Centre is an ethno specific, 152-bed, long-term care home located in Toronto. We are committed to achieving excellence in service delivery in a setting that is sensitive and respectful to the cultural and spiritual needs of everyone we serve.
Our interdisciplinary professional healthcare team includes: physicians, nurses, therapeutic recreation assistants, a dietician, a pharmacist, a recreation therapist and a social worker, who focus on the needs of our residents and improve quality of life in a culturally sensitive caring environment.
As an organization we value independence, self-sufficiency, the importance of family, maintaining active lives, and caring for one another, which underpin our philosophy of care.
Our barrier-free facility, with secure gardens and walking paths, courtyard, and atrium create a homelike environment for our residents. The on-site Melnyk Family Memorial Chapel hosts Catholic and Orthodox pastoral services. We support and encourage resident's own community clergy to visit and tend to individual spiritual needs.
Resident Care and Services Philosophy
The Ukrainian Canadian Care Centre is an ethnocentric long-term care home committed to honouring the diversity among our residents and to creating a caring environment. We are sensitive and respectful of cultural and spiritual needs of everyone we serve. Our resident-centred care and services philosophy acknowledges the uniqueness of each resident and aligns with our corporate Vision, Mission and Values, as well as Ontario's Residents' Bill of Rights.
We recognize and honour our residents' right for autonomy, to share their life stories and to maintain their chosen lifestyle. The resident and family, as core members of the interdisciplinary care team, are the primary contributors to the resident's individualized Plan of Care. Our interdisciplinary resident care and services team ensure that our innovative services are responsive and appropriate for each resident.
Our care and services philosophy is demonstrated through our integrated programs of care modalities, therapies and services that address the total well-being of each resident. Our team is committed to working with each resident to achieve his/her maximum potential. We encourage and support our resident's involvement within our Ukrainian Canadian Care Centre community as well as engagement in activities in our local community.
